Registration Information:
To complete the registration process, you must provide the following information:

Sponsor/spouse name, rank, duty station, duty phone number, AKO e-mail address and cell phone numbers.
Proof of income (All LES's, pay stubs, and income records for the family)
Two emergency contacts other than the sponsor and spouse. (must be local)
Current and UP-TO-DATE Immunization/shot records for children fifth grade and younger.
Physicals/health assesments for children fifth grade and younger. (Must be filed within 30 days)
Deployment Orders, if applicable
Military ID/DoD Card to show proof of eligibility
