If you need a New ID Card:
(You must provide TWO (2) Forms of ID)

*All CAC recipients must provide an official military e-mail (.mil) prior to the issuance of the ID Card.

SPONSOR MUST BE PRESENT FOR ALL TRANSACTION OR DEPENDENT MUST HAVE
CURRENT AND VALID GENERAL OR SPECIAL POWER OF ATTORNEY
